Based on a popular otome game, Hakuouki follows Chizuru and the Shinsengumi in an alternate reality. Chizuru travels to Kyoto in search of her missing father but after being involved in an incident, ended up being taken in by the Shinsengumi where she meets the members including the famous Hijikata Toshizo and Okita Soshi. Chizuru’s father was conducting government research for strengthening the Shinsengumi and developed the ochimizu. The ochimizu is a formula that gives demon like strength and regeneration effects but once taken, the user experiences extreme pain and eventually turns into a blood hungry monster.
In an attempt to defend Chizuru, Hijikata takes the ochimizu and although the situation was saved, he is now affected with the illness. Okita is also forced to take the formula when his tuberculosis prevents him from defending Chizuru and himself. By a strange turn of events, three acquaintances; an illustrator, an editor and a stage performer, end up investigating Otogai's odd behaviour. They discover that the writer's garden appears to be haunted by a mysterious camellia bush...and camellia was Uro's favourite flower.

In the world right now, there are creatures called "Extraneous" that appear out of nowhere, kill people, and disappear again. No one remembers when they started appearing, but the chance of being killed by one is about the same as the chance of dying in a car accident. Midori's mother was killed by one a year ago, and not only is her father rarely around, he's also deeply in debt. She gets attacked by an Extraneous on her way home, but is saved by a mysterious girl who claims that she's a god. Midori names her Hitsuji (sheep) because of her horns, and takes the girl in. They start a website, Hitsujiya, an Extraneous-fighting service provided by Hitsuji.

Among the mostly male human population, the light-haired elite class is allowed to temporarily keep the dark-haired "mongrels" as pets. One elite member, Iason, encounters a mongrel named Riki in the slums and decides to take him in. However, Iason keeps Riki longer than it is socially approved, and rumors abound about their possible relationship.
